What to bring to your DOT Physical and USCG Physical:
- If you wear hearing aids or glasses, please bring them to your exam
- Please bring a list of all your current medications and dosages.
- Blood pressure must be under 140/90. If you are currently taking blood pressure medication, remember to take it the day of your exam.
- Special circumstances: you must provide documentation from your primary care / specialist that includes a statement that you are “cleared to drive a commercial motor vehicle without restrictions” if you:
- Are currently taking ADHD medication, narcotics for pain or psychiatric conditions or sleeping pills
- Have heart-related issues (including cardiac bypass surgery, valve replacement, open heart surgery, pacemaker, or stent)
- Have sleep apnea and are using a C-Pap machine, also bring your most recent compliance report
- Have diabetes that is controlled by diet or oral medication, also bring your most recent blood work
- Have diabetes controlled with insulin, bring the Insulin Treated Diabetes Mellitus (ITDM) Assessment Form, MCSA-5870 https://www.fmcsa.dot.gov/regulations/medical/insulin-treated-diabetes-mellitus-assessment-form-mcsa-5870 completed by your primary care / specialist

- 1. Prior to a Captain's physical, go to to https://www.dco.uscg.mil/Portals/9/NMC/pdfs/forms/CG_719K.pdf to fill out the APPLICATION FOR MEDICAL CERTIFICATE (FORM CG-719K) and bring to your appointment.
- 2. USCG Certified Drug Test / DOT drug screening test is a urine test which is done according to the strict regulations required by the U S Coast Guard and the Department of Transportation (DOT) for testing and chain of custody licensing requirements. All DOT drug tests use the same 5-panel test. It tests for:
- Marijuana/THC
- Opiates (including codeine, heroin, morphine, hydrocodone, oxycodone, hydromorphone, and oxymorphone)
- Phencyclidine
- MOA-Analogues
- Cocaine
- 6-Acetylmorphine
- Amphetamines (including methamphetamine, MDMA)
- Oxycodones
